When someone goes overboard, there's no margin for hesitation. What happens in the first sixty seconds often decides whether the situation ends in a recovery or a tragedy and understanding why that window is so short is what makes the response instinctive instead of a scramble.

Why the first minute matters more than it seems like it should
The moment someone enters the water, several things start working against them at once. Distance from the vessel increases faster than most people realize, waves can quickly obscure a person from view, and stress or panic can make even simple tasks difficult. In rough conditions, someone can disappear between waves within seconds. Lose sight of them, and what started as a recovery can quickly become a search.
At the same time, the person in the water is fighting challenges of their own. Even in favorable conditions, treading water, staying oriented, and remaining calm takes energy. Fatigue can set in surprisingly quickly, especially if the person is injured, weighed down by gear, or struggling against wind, waves, or current.
In cold water, those challenges become even more severe. Sudden immersion triggers an involuntary gasp reflex and rapid, uncontrollable breathing known as cold shock. If that gasp happens underwater, from a bad entry or a wave at the wrong moment, water can be inhaled immediately. This initial response occurs within seconds and is often the most dangerous phase of cold-water immersion.
As exposure continues, hand and arm function begin to deteriorate as the body redirects blood flow toward the core. Tasks that seem simple on deck, grabbing a line, holding onto flotation, climbing a ladder, can become difficult or even impossible. This can happen long before hypothermia, the slower and more familiar threat, becomes a factor.
In most overboard emergencies, outcomes are determined by two things: whether the crew can maintain contact with the person in the water, and whether the person can stay afloat and responsive long enough to be recovered. That's why the response can't be something that's figured out in the moment. It must be automatic.
The first 60 seconds: what to do
While exact procedures vary by vessel, the fundamentals hold across recreational boats, commercial crews, and search and rescue operations alike:
- Shout and signal immediately. The first person to see the fall calls it out loud and specific, "Man overboard, port side!" not just "someone's in the water." A clear, specific call is what gets the rest of the crew moving without confusion.
- Assign one person to point and watch, and nothing else. Their only job is keeping eyes on the person in the water, continuously. Losing visual contact is one of the most common, and most dangerous, failures in an overboard situation, especially in chop or low light.
- Throw flotation immediately. A life ring, buoyant cushion, or dedicated throwable device goes in the water toward the person right away. Beyond the support it provides, it gives the person something to work toward and helps mark their position if they're momentarily lost from view.
- Mark the position digitally. Hit the MOB function on your GPS or chartplotter the moment it happens. If visual contact is lost, which, in real conditions, it often is, that waypoint is what makes a search possible instead of a guess.

Control the vessel. Cut speed and maneuver deliberately to avoid a propeller strike, then bring the boat around to begin recovery. Depending on your boat and conditions, that might mean cutting the engine, turning into the wind, or running a proper recovery pattern.
If you're the one in the water
Self-survival matters just as much as the crew's response. Stay calm and conserve your energy. Panic and unnecessary movement can lead to exhaustion surprisingly quickly, especially in rough conditions. If you have flotation, let it do the work. Thrashing and excessive swimming burn valuable energy and can make it harder to stay oriented and visible.
Keep your clothing on. Many people instinctively want to remove heavy clothing, but trapped air and fabric layers can provide additional buoyancy and insulation. Unless the clothing is creating a specific hazard, it's usually better left on.
If rescue isn't immediate, make yourself as easy to find as possible. Stay with the vessel, flotation device, or any large floating object whenever you can. Rescuers are far more likely to spot a person near something visible than a person alone in the water.
In cold-water conditions, take additional steps to conserve heat. Minimize unnecessary movement and, if possible, adopt a heat-conserving position (shown below) while awaiting rescue.

Train for the moment you can't predict
Overboard incidents are rarely dramatic leading up to the moment itself. They happen during routine tasks: line handling, a transfer, a moment of distraction, which is exactly why the response must be practiced rather than reasoned out in the moment. Crews that drill the shout, point, throw, mark, maneuver sequence respond faster. Crews that know their gear, what floats, what warms, and what each is for, recover more effectively.
